Managing and coordinating care in the home

The Medicaid Wavier Program is a comprehensive, community-based, in-home services program designed specifically for low income persons who are medically appropriate for nursing home placement.  The goal is to provide a client with a choice to remain independent in their home with services, instead of going to a nursing home.  The Waiver Program includes prescreening, assessment, and care management. Funding for services is provided through Michigan Department of Community Health.

Eligibility

Clients must live in Jackson, Hillsdale, or Lenawee County. Frail adults age 65 or older, and physically disabled who are 18 or older are the intended recipients of the Waiver program. 

  1. FINANCIAL:  To be eligible for the Waiver program, an individual’s income must be less than $1,911 per month (2008).  If the client has a spouse, the spouse’s income is not computed in the qualifying amount.  Allowable assets for an individual client are limited to $2,000.  If the client has a community spouse, the federal spousal impoverishment guidelines are utilized.
  2. MEDICAL:  Using a Level of Care Determination screen, eligible recipients must be appropriate for the same level of care needed as a nursing home resident. 
  3. SERVICE NEED:  Client’s must need at least one Waiver service to be eligible.

How does a person become a Medicaid Waiver client?

All persons referred to the program receive a telephone interview to assist in determining eligibility for an in-home assessment.  An individual not eligible for the assessment is assisted by a qualified Information and Assistance Specialist.  If the person is eligible for the assessment, a nurse and social worker visit the home for a detailed assessment.  The assessment evaluates the individual’s physical and mental health, functional status, informal and formal supports, home environment, financial resources, and other resources.  An eligibility specialist will submit the application for Medicaid and formal services.
