Managing and coordinating care in the home
For Frail or Disabled Adults: What happens when a loved one, such as a parent, spouse or grandparent, can no longer care for his or herself due to frailty or a disabling illness? Often, family caregivers believe that their only choice is a nursing homebut there are options.
Area Agency on Aging Care Management staff can help people sort through the options and arrange for the care that is needed. The goal of the program is to offer a realistic choice for receiving long term care in the home and community, rather than in a nursing home.
Care Management nurses and social workers are experienced in identifying needs, supporting family caregivers and managing community services.
Services Provided
For those who require extensive in-home help, a nurse and social worker team assess the health, emotional, social, financial and environmental conditions of individuals in order to prepare a plan of care. This plan includes home-delivered services such as meals, personal care, homemaker and relief for families. The client reviews the care plan and may accept or refuse any part of the plan.
Once this plan is in place, the team arranges the services to be delivered, monitors the providers and reevaluates the clients condition and makes adjustments as necessary. Services may be paid for in a number of ways including Medicare, private insurance, community grant programs, and Medicaid.
Eligibility
To receive services, care management clients must be 60 or older and live in Hillsdale, Jackson or Lenawee County.
